[Awclist] Updating Adobe Whitewater database

Hello all - the clinics are starting, the pool sessions are great places 
to warm up those muscles from winter slumber and we are seeing 28-30 
people at the monthly Albuquerque meeting (the food is also awesome).

This is also dues times, many of you are actually paid up, I will be 
sending out invoices in the next week or so.

AWC is re-starting the membership directory and we would like to have 
you fill out the attached membership form (look closely it has changed) 
so that we can update our database and send out current and accurate info.

There are places where you can opt out of the directory and/or the list 
serve, just mark these boxes if you DO NOT want to be included.

Thank you all for your continued support of the Club, its great to see 
everyone.
